LAWPRO® CPD PREMIUM CREDIT - Declaration

To ensure consistency with the Law Society's terminology, the CLE Premium Credit will be known as the CPD
Premium Credit effective September 16, 2010. All other aspects of this program will remain unchanged.

The deadline for claiming a CPD Premium Credit for your 2013 premium is September 15, 2012.
The online declaration form appears below, and must be filled out by this deadline for you to receive the credit. You are
eligible to claim a 2013 premium credit of $50 per program (to a maximum of $100) for attendance at approved
programs offered between September 15, 2011 and September 15, 2012.
You can claim the credit for reviewing an approved program on an archived webcast, CD-ROM, audio tape or video replay,
provided you watch or listen to the entire program and have a copy of the program materials. On the CPD declaration form
you should claim credit for a self-study review.
You are also eligible for the Premium Credit if you chair or speak at a qualifying program, provided you attend the
entire program. The Premium Credit is applicable to Standard coverage, and cannot be applied to Run-off or Excess policy
premiums.
Approved programs offered between September 15 2011 and September 15 2012 will be eligible for the 2013 Premium Credit. CPD Premium Credits may not be carried forward. Unclaimed credits for attendance at qualifying programs offered before September 15, 2011 are now lost.

To be eligible for the CPD premium credit (to be applied to the 2013 liability insurance premium for Ontario lawyers only), please complete this DECLARATION form.
You must fill in all the information requested on this form.
The CPD Premium credit will automatically appear on the invoice you will receive with your 2013 insurance policy.
